Some insurance plans have narrow networks and we always recommend calling the phone number on your insurance card prior to your appointment to verify if we are in your specific network.
Please contact your insurance carrier to determine if a referral is needed from your primary care physician. Referrals are the member’s responsibility. If your claim is denied because your referral was not obtained before your appointment, you will be responsible for the cost of the visit.
