Preparing for Your Appointment
Please bring the following information with you:
- Your health insurance card (if we cannot verify your coverage at the time of your appointment, you will be responsible for all charges at time of service)
- Photo identification
- A current medication list with name/dose/strength/amount included
- Copayment required by your health insurer
Market Street Dermatology accepts the insurance carriers listed below. Please contact your insurance carrier prior to making an appointment if your insurance is not listed. Some insurance plans have narrow networks and we always recommend calling the phone number on your insurance card prior to your appointment to verify if we are in your specific network.
Please contact your insurance carrier to determine if a referral is needed from your primary care physician. Referrals are the member’s responsibility. If your claim is denied because your referral was not obtained before your appointment, you will be responsible for the cost of the visit.
